Provides culturally competent advanced medical social work services to DHEC clients/families in an interdisciplinary clinical setting.These specialized services are directed toward identifying and resolving the social, emotional and/or environmental risks affecting health or social functioning or interfering with the client's response to treatment or the need for care. Provides information on accessing resources and assists with reducing barriers to care.
Provides care coordination services for assigned CYSHCN caseload, Newborn Sickle Cell Surveillance and First Sound Programs. Remains current with care coordination practices.
Demonstrates social work competencies which include but are not limited to: conducting psychosocial assessments, providing intervention according to the plan of care through home and office visits and telephone contacts, and providing comprehensive and individualized discharge planning. Documents social work services in accordance with practice, agancy and program standards.
Coordinates with other community agencies, programs and services and makes referrals to enhance the psychosocial functioning of the client/family. Promotes resource development as appropriate.
Participates in interdisciplinary team meetings and provides consultation and coordination of services with other disciplines. Assists the team in understanding the social and emotional factors related to the client's medical condition.
Participates in regular professional supervisory conferences, region social work meetings, program staff meetings and other region/program meetings as appropriate.
Participates in staff development/inservice programs which contribute to professional knowledge and skills.
Participates in an emergency or major disaster to expedite services to the public. During such times, the incumbent should regard themselves as being on 24 hour call and subject to duty when such events occur or are anticipated to occur. Other duties as assigned.
